Quality care with a gentle touch
We offer caring and progressive rehab options for residents requiring short-term skilled nursing rehabilitation. Our Rehab to Home program features tenured PT, OT and ST staff, with program oversight by Orthopedic Surgeon, Dr. Riccardo Giovannone, DO. Dr. Giovannone conducts medical rounds on ortho-rehab patients at our facility and has provided guidance and equipment selection for our state-of-the-art rehabilitation department.Our on staff dedicated pulomonologist, Dr. William C. Decker, DO and Medical Director Jonna Schmidt, MD ensure that your loved one has the attention they may need without delay. With privileges at Promedica Bixby and Herrick Hospitals and 53 years of joined experience you can rest assured that your loved one is in the best hands.
For patients requiring additional medical care, such as oxygen, wound care, and IV management, we offer a full range of skilled nursing services in a caring and supportive environment. With our newly remodeled rehabilitation wing, which includes flat screen TVs and free cable, your stay will be comfortable and enjoyable.

Karen Porter - Poffenberger from Jackson was admitted to Lynwood Manor after a lengthy hospital stay. Notice of Data Security Incident
Adrian, Michigan – CRG Lynwood, LLC d/b/a Lynwood Manor (“Lynwood”) experienced a data security incident that may involve the protected health information of some individuals it serves. Lynwood takes the privacy and security of information in its possession very seriously and sincerely apologizes for any inconvenience this incident may cause. This notice is intended to alert potentially impacted individuals of the incident, steps we are taking in response, and resources available to assist and protect individuals.
What Happened: On July 12, 2021, Lynwood discovered it was the victim of a data security incident that impacted its network environment. Immediately following, Lynwood engaged with a specialized, third party cybersecurity forensic firm to determine the nature and scope of the incident. The investigation, which concluded in November, 2021, revealed that an unauthorized party gained access to Lynwood’s network. Therefore, since December, 2021, Lynwood began working with a data mining vendor to determine whether individuals’ protected health information was impacted. In September, 2022, Lynwood discovered protected health information of its constituents located in the systems impacted by the underlying cyber-attack. The process of identifying the specific individuals affected by the underlying incident remains ongoing. While Lynwood has no reason to believe that any individual’s information has been misused as a result of this event, Lynwood is providing notice out of an abundance of caution.
What Information Was Involved: While we have no reason to believe that individuals’ information has been misused as a result of this incident, we are notifying out of an abundance of caution and for purposes of full transparency. Based on the ongoing investigation, the unauthorized party may have had access to individuals’ names and medical information.
What We Are Doing: The security and privacy of resident information contained within Lynwood’s systems is a top priority, and Lynwood is taking additional measures to protect this information. Since the incident, Lynwood has continued to strengthen its security posture by adding the following security controls: implementing multi-factor authentication; conducting cybersecurity awareness training, improving technical security measures, and took steps and will continue to take steps to mitigate the risk of future harm.
In light of the incident, notification letters will be sent to those individuals with the further information regarding the underlying incident once the data mining process is complete.
What You Can Do: Lynwood encourages all individuals to remain vigilant against incidents of identity theft and fraud, to review their account statements, and to monitor their credit reports for suspicious or unauthorized activity. Additionally, individuals should contact their financial institution and all major credit bureaus to inform them of the incident and then take whatever steps are recommended by these institutions, which may include placing of a fraud alert on the individual’s account.
